What happens if we don't turn in an evaluation?

Not submitting an evaluation can jeopordize your current or future funding. If you have any questions please contact the ZAP program office at 801 468-3517.

Does contract personnel include performance contracts or just people who are helping implement a project such as a musical?

Both

Are people who are performing in a musical or playing in the pit for free considered a volunteer or should I list them under artists?

You may list them in both places.

If children are performing as part of the Shakespeare Festival or other musical program, are they considered "artists".

I'm a bit confused about the submission procedure. You want the electronic version AND hardcopy? But the electronic version is optional?

The electronic version helps me to cut and paste your language onto our website or for publications. Some organizations may not have access to the internet, so it is optional for Tier II organizations. The hard copies are needed to distribute to our advisory board members as they review your application. The advisory board gets your application and the last evaluation that I have on file

The reporting cycle on my Tier II Actual Use Evaluation Report says that it must be between Jan 1, 2008-December 31, 2009. Wouldn't that be the cycle we are in? I am pretty sure that the last evaluation report I sent in was for September 1, 2006-August 31, 2007, so should I send this report in for September 1, 2007-August 31, 2008?

Currently, each Tier II organization determines its own 12 month reporting period based on how you completed your application. Go find your 2007 application (if you are doing the 2007 evaluation), look on the first page where it asks for the time period. That is the period for which you are reporting.

I just took a quick look at the Tier I and Zoological online evaluation form and wonder if Narrative Question #3 and #5 are  asking the same question?


Question 3: Describe what was accomplished using your ZAP funding and how ZAP funds assisted with your organization’s goals? If applicable, include a list of free days or events, and how they were publicly announced.

Question 5: Describe any noteworthy achievements and awards that occurred during this cycle using ZAP funds.

Number 3 and 5 do seem very similar. The difference is…

Question 3 – is directly related to the use of your ZAP funding

Question 5 – could be any noteworthy achievement like when the Shakespearean Festival got a Tony award – so it may not be related to ZAP funds, it could be a great review, acknowledgement by a corporation or foundation, increase in some special grant, noteworthy achievements of your staff, actors, playwrights, etc.
